1. Informational Purposes Only; No Doctor-Patient Relationship

The Sites provide general information about dental services offered by Denovo. Nothing on the Sites is dental or medical advice, and viewing the Sites, submitting a form, or booking a consultation does not create a doctor-patient relationship. A doctor-patient relationship is established only after you have been examined by a Denovo provider and accepted for treatment.

2. Pricing, Promotions, and the All-On-4 Package

Prices shown on the Sites, including the $9,999 All-On-4 package, are advertised starting prices for qualifying patients and are subject to the following:

• Consultation and exam required. Whether you are a candidate for the advertised treatment, and the final fee, can only be determined after an in-person examination, 3D imaging, and review of your health history by a Denovo provider.
• What is included. The advertised package includes a full upper or lower arch of zirconia teeth secured to four to six dental implants, with additional implants included at no extra charge when clinically indicated by your Denovo provider. Any items included or excluded beyond this are described on the Site and confirmed in your written treatment plan.
• Additional treatment. Some patients require additional procedures before or alongside the advertised treatment (for example, extractions, bone grafting, sinus lifts, sedation, or treatment of the opposing arch). If any such procedures are needed and are not part of the advertised package, they will be presented to you in writing with their cost before you commit to treatment.
• Per arch. Advertised pricing is per arch unless stated otherwise.
• Comparison pricing. References to what All-On-4 or full-arch treatment costs at other offices (for example, "$15,000–$25,000+") are based on publicly available pricing and industry estimates. They are provided for context only. Prices at other providers vary and are not verified by Denovo. Stated "savings" are calculated against these comparison figures and are not a guarantee of your individual savings.
• Financing. Monthly payment examples (for example, "$350/month") are illustrative and depend on the financing partner, approved amount, term, and your credit. Financing is offered through third-party lenders and is subject to their approval and terms. Denovo does not guarantee financing approval.
• Insurance. Advertised prices are cash/financed prices and may not be combinable with insurance benefits or other offers.
• Changes. We may modify or withdraw any promotion at any time. The price quoted in your written treatment plan governs.

3. Results Vary

Dental treatment outcomes depend on individual factors including bone quality, oral health, medical history, healing, and compliance with post-operative instructions. Before-and-after photographs on the Sites show actual Denovo patients who consented to their use; their results do not guarantee yours. Testimonials and reviews reflect individual experiences and are not a promise of similar results.

4. Appointments and Bookings

• Online bookings made through the Sites are requests for a consultation. A booking is confirmed only when you receive confirmation from our office by phone, text, or email.
• We may contact you by phone, text, or email to confirm, reschedule, or follow up on your request. See our Privacy Policy for how we use your contact information and how to opt out.
• Please provide accurate and complete information when booking. We may cancel bookings made with false or incomplete information.
• Our cancellation and no-show policy will be provided to you at the time your consultation is confirmed.

5. Text Messaging Terms

By providing your mobile number and opting in, you agree to receive text messages from Denovo related to your inquiry, appointments, and treatment options. Message frequency varies. Message and data rates may apply. Reply STOP to cancel and HELP for help. Consent is not a condition of purchase. Carriers are not liable for delayed or undelivered messages.

6. Acceptable Use

You agree not to:

• Use the Sites for any unlawful purpose.
• Submit false, misleading, or fraudulent information or book appointments you do not intend to keep.
• Attempt to gain unauthorized access to the Sites, their servers, or connected systems.
• Scrape, copy, or reproduce Site content for commercial purposes without our written permission.
• Interfere with the operation or security of the Sites.

7. Intellectual Property

All content on the Sites, including text, graphics, logos, photographs, videos, and page design, is owned by or licensed to Denovo and is protected by copyright and trademark law. You may view and print pages for your personal, non-commercial use only. Any other use requires our prior written consent.

8. Third-Party Services and Links

The Sites use third-party services (for example, online booking, financing applications, maps, and review platforms) and may link to third-party websites. We do not control and are not responsible for the content, availability, or practices of those third parties. Your use of them is governed by their own terms.

9. Disclaimer of Warranties

The Sites are provided "as is" and "as available." To the fullest extent permitted by law, Denovo disclaims all warranties, express or implied, regarding the Sites, including warranties of accuracy, completeness, availability, and fitness for a particular purpose. We do not warrant that the Sites will be uninterrupted or error-free.

10. Limitation of Liability

To the fullest extent permitted by law, Denovo and its owners, providers, employees, and contractors will not be liable for any indirect, incidental, consequential, special, or punitive damages arising out of or relating to your use of the Sites. This section does not limit any liability that cannot be limited under applicable law, and it does not apply to claims arising from dental treatment you actually receive from Denovo, which are governed by your treatment agreements and applicable law.

11. Indemnification

You agree to indemnify and hold harmless Denovo from any claims, losses, or expenses (including reasonable attorneys' fees) arising from your violation of these Terms or your misuse of the Sites.

12. Governing Law and Venue

These Terms are governed by the laws of the State of Washington without regard to conflict-of-law principles. Any dispute relating to the Sites will be brought in the state or federal courts located in King County, Washington, and you consent to the jurisdiction of those courts.

13. Changes to These Terms

We may revise these Terms at any time by posting an updated version with a new effective date. Your continued use of the Sites after changes are posted constitutes acceptance of the revised Terms.

14. Severability and Entire Agreement

If any provision of these Terms is found unenforceable, the remaining provisions remain in effect. These Terms, together with the Privacy Policy, are the entire agreement between you and Denovo regarding use of the Sites. They do not replace any treatment consent, financial agreement, or other document you sign as a patient.
